New Patient Form:

New patients can save time during their first appointment by completing the New Patient Form prior to their visit. Simply print out the New Patient Form, fill in the information requested, and bring the completed form with you to your appointment.

Make A Referral

For service providers:   Making a referral is quick and easy.  Simply fill out the online referral form or the attached referral  form and fax to (651) 738-1881. The referral will be processed within 1-2 days and the referral source will be informed of the outcome of the referral.  

Crisis
For mental health emergencies, please call 988. You will be directed to the closest mental health crisis triage team in your area.  Additional phone numbers for your respective counties are also listed below.
